Yizkor Book Submissions 2025

The Yizkor Book is a sacred memorial book that is read during Yizkor services, which are held on Yom Kippur, Shemini Atzeret, Pesach and Shavuot. Yizkor, meaning "remember" in Hebrew, is a solemn and meaningful moment when we honor the memory of our departed loved ones. We invite you to contribute to the Yizkor Book by submitting the names of your loved ones for inclusion. You may also consider including names of relatives or friends who no longer have loved ones to honor their memory. A minimum contribution of $18 per name is requested.

Every effort will be made to ensure accuracy, as we understand the importance of honoring your loved ones' memory and want to make sure their names are included correctly. We appreciate your generosity and support of this tradition.


Submissions are due by August 15, 2025.
How Names Are Listed in the Yizkor Book
Names will appear exactly as entered in the form below.
Listings will follow this format: three columns—

In Memory ofRemembered by, and Relationship.
Example: Sharon Lastname wants to list her Mother, Mildred 
Sharon will fill out the form as follows:
  • Full Name of Deceased: Mildred Lastname
  • Relationship: Mother
  • Mourner’s Name: Sharon Lastname
