Where has the student attended Hebrew School and /or received a Jewish education?*

*Students must be enrolled in the COS Hebrew School, the Jewish Academy of Orlando and/or another Hebrew School or Jewish day school approved by the COS Rabbi for at least 5 years prior to the Bar/Bat Mitzvah, or they may not be eligible for a Shabbat morning Bar/Bat Mitzvah in the synagogue.

COS assigns Bar/Bat Mitzvah dates based on the proximity to the student's 13th Hebrew birthday. COS will strive to meet specially requested dates if the date is available but does not guarantee that all requests will be accommodated. Please note any specially requested dates below:

The Bar/Bat Mitzvah fee is currently $1350.00 and includes training, books/materials/recordings, and standard building maintenance fees. Please note that the Bar/Bat Mitzvah fee does not include any COS kitchen or outside catering fees or any additional maintenance and/or security fees that may be required based on extraordinary facility requirements for your event.

The Bar/Bat Mitzvah fee must be paid via monthly by ACH and/or credit card payment such that fifty percent (50%) of such fee is paid no later than six (6) months before the Bar/Bat Mitzvah, and the remainder of such fee is paid no later than two (2) months before the Bar/Bat Mitzvah. COS will contact you to set up recurring payments when lessons begin. If you wish to begin paying the fee at an earlier time, recurring payments can be set up through your member account on ShulCloud at ohevshalom.org or by contacting our Executive Director at execdir@ohevshalom.org. All fees are subject to change prior to May 31st of the student's sixth grade school year.

Please fill out this completed form with the $150.00 required deposit below, which will be applied to the final Bar/Bat Mitzvah fee payment. Bar/Bat Mitzvah dates will not be assigned without this deposit. You will be notified of your student's assigned date when it is approved by bookkeeping and the clergy.
